Free 12-Week Program
A physician-led 12-week program for type 2 diabetes reversal
We reviewed LifestyleRx carefully before recommending it to our community. Here’s what our team found — including an honest conversation about what “reversal” actually means.

Why We’re Sharing This
At Diabetes Care Community, many of our readers ask whether meaningful improvement in type 2 diabetes is possible — even after years of living with the condition.
While there is no “quick fix,” research shows that targeted lifestyle changes can significantly improve blood sugar levels and, in some cases, lead to remission when done under medical supervision.
Covered by your provincial health plan. If you live in BC, Alberta, or Ontario, the LifestyleRx program is covered — meaning there is no cost to you to participate in a structured, physician-supervised 12-week diabetes program. This is not a private-pay service.
About the program
What is the LifestyleRx 12-week program?
The program is delivered virtually by Canadian family physicians and specialists, covered by provincial health plans in BC, Alberta, and Ontario and is focused on structural lifestyle change with medical oversight.
The program is based on clinical research suggesting that type 2 diabetes is driven by interrelated metabolic processes — particularly insulin resistance — that can improve through targeted lifestyle intervention.
A note on the word “reversa”: LifestyleRx uses this term, and we want to address it directly. In clinical practice, reversal refers to achieving sustained blood sugar levels in the non-diabetic range without the need for certain medications — often called remission. Research shows this is achievable for many people with type 2 diabetes through significant lifestyle change. It is not a cure, and maintaining results requires ongoing commitment. LifestyleRx’s program materials reflect this distinction accurately, and so does this page.
Program contents
What the 12 weeks covers
The program focuses on six key lifestyle behaviours — the same ones clinical research has identified as most effective in reducing insulin resistance and improving metabolic health.
What to eat — and when
Practical, whole-food nutrition guidance
focused on meal composition and timing —
not calorie counting. Common-sense
principles you can maintain after the program
ends
Sustainable physical activity
Walking, cycling, or whatever movement you
can maintain. The program asks only for
exercise that feels manageable — not extreme
routines you’ll abandon in week three.
Sleep improvement
Sleep quality has a direct effect on blood
sugar and insulin sensitivity. The program
addresses it as a metabolic lever, not an
afterthought.
Stress management
Chronic stress elevates cortisol, which raises
blood sugar. Structured stress reduction is
integrated into the program alongside dietary
and activity changes.
Physician oversight throughout
Delivered by Canadian family physicians and
specialists who monitor your progress, guide
medication decisions where relevant, and
provide medical supervision for the full 12
weeks.
Metabolic health tracking
Structured monitoring of blood sugar, A1C
trajectory, and other metabolic markers so
both you and your physician can see what’s
working across the 12 weeks.
Covered in BC, Alberta & Ontario — at no cost to you.
Enrolment takes a few minutes and there’s no commitment required beyond the program itself.
How the 12 weeks unfolds
What to expect week by week
Foundation: Weeks 1 – 3
Assessment, baselines & first changes
Your physician establishes your metabolic baseline — blood sugar, A1C, insulin resistance markers. You’re introduced to the nutrition and meal timing framework. First lifestyle changes are implemented gradually, under medical supervision.
Core Program: Weeks 4 – 8
All six behaviours integrated
The full program is in motion — diet, activity, sleep, and stress management working together. Your physician monitors progress and adjusts guidance based on your results. For some participants, early metabolic improvements become visible at this stage.
Consolidation: Weeks 9 – 12
Building lasting habits
This phase focuses on cementing the behaviours you’ve built into durable, long-term habits. Your results are reviewed comprehensively and a personalised maintenance path is mapped out.
On medications
What happens to your medications
Some people with type 2 diabetes who achieve significant blood sugar improvement through the program are able to reduce or discontinue certain diabetes medications. This happens only under physician supervision — it is never self-directed. LifestyleRx physicians monitor participant progress throughout all 12 weeks and guide any medication decisions as part of the program. Diabetes Care Canada strongly encourages all readers to speak with their own healthcare provider before starting any new program, including this one.
